The best way to document your Design Reviews meeting and action items summaries will be to create a DOC template in your templates repository that will be used when ever you are having a meeting. The DOC structure can be as suggested in this illustrated image taken from our recent implementation project for one of our Medical Device customers.
Note the once chapter in the document is dedicated to log the meeting objective and agenda, all other topics are action items given to the meeting participants which can be control and governance.
